ABSTRACT

Objective To investigate the risk factors for ischemic stroke in Uygur population in Xinjiang, China. Methods Using a case-control study mode, 504 Uygur patients with first-ever ischemic stroke and 507 Uygur healthy subjects w ere investigated in accordance w ith the principle of the ethnic, sex, and age matching. The risk factors for ischemic stroke w ere screened. Results Multivariate logistic regres-sion analysis showed that the obesity (odds ratio [OR] 4.82, 95% confidence interval [CI] 1.80-12.94;P=0.002), hypertension ( OR 8.20, 95%CI 4.93-13.51; P<0.001), and heart disease ( OR 2.40, 95%CI 1.07-5.39;P=0.033) w ere the independent risk factors for ischemic stroke in Uygurs. While the education level of junior high school or above ( OR 0.38, 95%CI 0.18-0.81;P=0.012), tea drinking habit ( OR 0.22, 95%CI0.06-0.70; P=0.021), siesta habit ( OR 0.38, 95%CI 0.24-0.59; P<0.001), high levels of high-density lipoprotein cholesterol ( OR 0.34, 95%CI 0.20-0.59; P<0.001), and apolipoprotein A ( OR 0.23, 95%CI 0.07-0.77; P=0.017) were the protective factors for ischemic stroke. Conclusions The risk factors for ischemic stroke are more and typical in Uygur population in Xinjiang, China. Adjusting the diet structure of Uygurs, less taking meats and high-salt diet, intaking high fiber foods and fish, and appropriate physical exercise w il play an important role for the prevention of stroke in Uygur population.

ABSTRACT

Objective To discuss the improvement of surgical techniques and adjustment of immunosuppressive regimen for combined liver and intestinal transplantation.Method A male patient with liver dysfunction and short bowel syndrome underwent the combined liver and intestinal transplantation.Ostomy of graft was performed instead of intestinal anastomosis during the operation.The anastomosis of graft and autologous intestine was performed 8 months after transplantation.Hospital and follow-up data of the patients were analyzed retrospectively.Result The functions of liver and small bowel recovered smoothly after operation.Slight rejection occurred one month after operation with normal function of intestine but dysfunction of liver.In the first month after operation, abdominal infection was controlled by intraperitoneal drainage with open surgery.Immunosuppression protocol was administrated with alemtuzumab for induction plus maintenance treatment with tacrolimus, and mycophenolate mofetil was added because of renal dysfunction 2 years after transplantation.The patient was followed up for nearly 3 years with good quality of life without rejection and infection.Conclusion Combined liver and intestinal transplantation could improve patient's life quality and extend the survival time through the improvement of surgical techniques and individual immunosuppressive regimen.

ABSTRACT

Objective To study the pretreatment with prostaglandin E1 ( PGE1 ) on acute superior mesenteric ar-tery ( SMA) ischemia reperfusion injury of intestinal cell in rats. Methods 18 healthy male SD rats were randomly divided into control group, sham operation group and experimental group. IRI of SMA model was made by clamping the SMA for one hour and two hours after reperfusion in the control group and the experimental group,respectively. PGE1(20 μg/kg) was injected from the tail vein in the control group and the experimental group. Character of pa-thology of small intestine was examined. The expression of Bax and Bcl-2 in small intestine cells and change of IF-ABP and DAO in serum were detected. Results Pathologic changes showed that there was no change in the control group;while in the sham operation group,chorionic epithelium mucosa ulceration and hemorrhage and necrosis oc-curred more seriously than that in the experimental group. Expression of Bax and Bcl-2 was higher in the sham op-eration group and the experimental group than that in the control group(P<0. 05), and it was obviously lower in the experimental group than in the sham operation group(P<0. 05). The content of IFABP and DAO in blood ser-um:it was higher in the sham operation group and the experimental group than in the control group ( P<0. 05 ) , and it was lower in the experimental group than in the sham operation group(P<0. 05). Conclusion PGE1 can relieve the alvine necrosis caused by rats' mesenteric reperfusion injury after acute artery ischemia and thus protect damaged mucosa of small intestine.

ABSTRACT

Objective To investigate the effects of treatment on end-stage liver disease and diabetes mellitus by simultaneous liver-pancreas-duodenum transplantation with the pancreas of the recipients reserved.Method Simultaneous liver-pancreas-duodenum transplantations were carried out in three patients with the pancreas of the recipients reserved.The diseases of the recipient 1,2,and 3 were alcoholic liver cirrhosis and diabetes mellitus,chronic hepatitis B liver cirrhosis and diabetes mellitus,and chronic hepatitis B liver cirrhosis and diabetes mellitus complicated with renal function failure.The recipient 3 received simultaneous renal transplantation.Result The recipient 1 suffered from pancreatitis after the operation and discharged with normal liver function and blood glucose levels,and he was treated with insulin at 4th year after the operation.Intestinal fistula occurred in the recipient 2 and drainage was done without acute peritonitis,the liver allograft was experienced an acute rejection episode treated by intravenous bolus methylpredisolone at 19th month after operation,but gastrointestinal perforation happened and the patient died of acute peritonitis.In the recipient 3,peripancreatic effusion and pancreatitis happened and were treated by drainage,and the recipient survived to now with normal liver and kidney functions,but given insulin at first year after operation.Conclusion It is effective to implement simultaneous liver-pancreas-duodenum transplantation with the pancreas of the recipients reserved on the patients with end-stage liver disease and diabetes mellitus.However,how to maintain the pancreatic endocrine function after the transplantation for a long period awaits further investigation.

ABSTRACT

Objective To discuss the surgical procedures and treatment after combined liver and intestinal transplantation with portal venous drainage and enterostomy of two ends in one case.Methods A male patient with liver dysfunction and short bowel syndrome underwent the combined liver and intestinal transplantation.With the techniques of en bloc,the liver and intestinal grafts were harvested from cadaveric donor.The intestinal graft,200 cm long,was implanted with portal venous drainage and aortic inflow,and enterostomy of both ends was performed instead of intestinal anastomosis.The liver graft was placed in a piggyback fashion with end to end anastomosis of the bile ducts without T tube. Inmunosuppression protocol was administrated with campath-1H and tacrolimus.Endoscopic biopsy of intestinal graft was performed regularly,and clinical observation was done to monitor the acute rejection.Results In the first month after operation,abdominal infection was controlled by intraperitoneal drainage with open surgery.One suspect acute rejection was treated with methylprednisolone.Until sixth month,the functions of liver and intestine were progressively restored.However,the patient lost weight and could not be free from intravenous nutrition because of diarrhea.Conclusion Combined liver and intestinal transplantation with portal venous drainage and enterostomy of two ends is a simple surgical procedure with lower risk of surgical complications.This method is propitious to monitoring rejection and function improvement of the grafts.Diarrhea and loss of digestive juice are the main reasons of low body weight and malnutrition.

ABSTRACT

ObjectiveTo investigate the correlation between PTEN and p27 expression in gastric carcinoma. MethodsImmunohistochemistry was applied to examine the expression of PTEN and p27 in 49 cases of human gastric carcinoma,49 cases of tumor-side ward mucosa and 13 cases of normal gastric mucosa.ResultsPositive expressive rates of PTEN and p27 protein in colorectal carcinoma were 61.22 %(30/49)and 57.14 % (28/49),which were lower than those in tumor-side ward mucosa [91.84 %(45/49) and 93.88%(46/49)]and normal colorectal mucosa [100 %(13/13)and 100 %(13/13)](P< 0.05). The expression of PTEN and p27 in gastric carcinoma correlated with each other (r=0.580,P=0.005),and which were associated with degree of differentiation of carcinoma,PTNM stage and lymph node metastasis of the tumors.Conclusion The abnormal expression of PTEN and p27 participate the malignant conversion process of the gastric mucosa cell and the expression levels of PTEN and p27 protein may be used to evaluate the pathologic biology behavior of gastric carcinoma.

ABSTRACT

This study examined the role of regulated upon activation normal T cell expressed and secreted (RANTES) and its receptor C-C chemokine receptor type 5 (CCR5) in gastric cancer metastasis and the associated mechanism. The expression of RANTES and CCR5 was detected by using immunohistochemical staining and Western blotting in the gastric cancer tissues obtained from 60 gastric cancer patients with or without lymph node metastasis (n=30 in each). The results showed that the expression levels of RANTES and CCR5 were higher in gastric cancer with lymph node metastasis than in that without metastasis (P<0.05). The expression levels of RANTES in 30 lymph nodes with cancerous invasion were higher than in 30 normal lymph nodes (P<0.05). Chemotactic test revealed that the number of migrating gastric cancer cells (n=295.0±54.6) induced by the protein of cancer-invading lymph nodes was greater than that by the protein mixture from cancer-invading lymph nodes and RANTES antibody (n=42.5±11.6) (P<0.05). RT-PCR showed that the expression levels of the main Th1 cytokines (IL-2, Γ-IFN) were lower in gastric cancer with lymph node metastasis (2.22±0.90, 3.26±1.15 respectively) than in that without metastasis (3.07±1.67, 4.77±1.52 respectively) (P<0.05), but the expression level of the main Th 2 cytokine (IL-10) was higher in gastric cancer with lymph nodes metastasis (6.06±2.04) than in that without metastasis (4.88±1.87) (P<0.05). It was concluded that RANTES and its receptor CCR5 may contribute to gastric cancer metastasis through influencing the balance of Th1/Th2. RANTES and CCR5 may become a marker of gastric cancer metastasis.

ABSTRACT

OBJECTIVE To investigate the distribution and antibacterial resistance of nosocomial infection from a new hospital pathogens,and provide reference for clinical rational use of drugs.METHODS Statistical methods were used to analyze the data of pathogen′s formation,distribution and antibacterial resistance of a new hospital.RESULTS A total of 1852 clinical isolates were collected from Mar 2005 through Mar 2007,of which Gram-negative microorganisms and Gram-positive cocci accounted for 71.0 % and 29.0%,respectively.The most commonly encountered pathogens were Pseudomonas aeruginosa,Escherichia coli,Klebsiella spp,Acinetobacter,Staphylococcus aureus and coagulase-negative Staphylococcus.Except Acinetobacter,up to 6% strains of Gram-negative microorganisms were resistant to the imipenem,meropenem and piperacillin/tazobactam;Gram-positive cocci were still highly sensitive to vancomycin and teicoplanin.CONCLUSIONS A new hospital is similar with other hospitals in the data of formation,distribution and antibacterial resistance of nosocomial infection pathogens.

ABSTRACT

<p><b>OBJECTIVE</b>To study the changes of immune function in liver cirrhosis patients after splenectomy combined with resection of hepatocellular carcinoma (HCC).</p><p><b>METHODS</b>Sixteen patients with HCC associated with liver cirrhosis were divided into two groups: splenectomy combined with hepatectomy (n = 7) and hepatectomy (n = 9). T-lymphocyte subsets such as CD4, CD8, CD4/CD8 and Th-lymphocyte cytokines such as IFN-gamma, IL2, IL10 in 7 ml peripheral venous blood before operation and two months after operation were examined and compared between the two groups.</p><p><b>RESULTS</b>There was no significant difference in pre-operative CD4, CD4/CD8, IL2, IFN-gamma, IL10 levels in the two group. Two months after operation, the levels of CD4 (38.2% +/- 3.7%), CD4/CD8 (1.7 +/- 0.3), IFN-gamma [(104.4 +/- 14.9) pg/ml], IL2 [(98.6 +/- 18.6) pg/ml] were increased and those of CD8 (23.7 +/- 13.7) pg/ml and IL10 [(55.5 +/- 11.2) pn/ml] were decreased in the two groups, but the changes in the group of splenectomy combined with hepatectomy were more obvious than those in the hepatectomy group. The levels of CD4 (32.5% +/- 4.0%), CD4/CD8 (1.1 +/- 0.1), IFN-gamma [(70.5 +/- 12.6) pg/ml], IL2 [(80.9 +/- 13.5) pg/ml] in the group of splenectomy combined with hepatectomy, were much higher than those in the hepatectomy group; those of CD8 (29.4% +/- 4.0%), IL10 [(89.4 +/- 10.0) pg/ml] level were significantly lower than those in the hepatectomy (P < 0.05).</p><p><b>CONCLUSION</b>Splenectomy combined with hepatectomy for HCC associated with liver cirrhosis donor decrease but promote the recover T-lymphocyte subsets and Th1/Th2 cytokines from imbalance and improve the patient's antitumor immune function.</p>

ABSTRACT

Objective To study and summarize the cause and reoperative method of massive hemorrhage of upper gastrointestinal tract after gastrectomy. Methods The clinical data of 11 cases patients with massive hemorrhage of the upper gastrointestinal tract after gastrectomy from August 1986 to June 2000 were analyzed retrospectively. Results 5 cases were anastomotic bleeding,3 cases injured gastric remnant mucosa bleeding,2 cases ulcer bleeding after bancroft gastrectomy, 1 case overlooked leiomyoma bleeding.All patients were cured with reoperation.Conclusions To prevent post operative bleeding is the key. The evaluation of the condition of patients with massive hemorrhage of the upper gastrointestinal tract,timing of reoperative intervention,and the selection of proper operative method are also very important.
